2022/2023 Season Registration and Parent information

If you’re a new player to MDMHA the following must be completed before your player steps on the ice.
1 – The Parents Respect In Sport On-Line course through the OMHA. A one-time fee is not covered by MDMHA The course can be found at www.omha.net - Respect Tab - Education - Respect in sport - Parent program
2 – All new registrations must send a copy of the players birth certificate to [email protected] 3- Residential proof will also be required to be uploaded into the HCR when you sign up for MDMHA for the first time. Without this, your registration will not be complete.

NSF Cheques: $75.00 Service Charge
Administration Fee will be charged on all returned cheques or stop payments.
Players will be unregistered until all outstanding amounts have been paid in full. In addition, all ice privileges will be suspended until arrangements are made to replace outstanding fees.
Outstanding fees must be replaced in CASH/CERTIFIED CHEQUE, MONEY ORDER or ETRANSFER ([email protected]) for the returned cheque plus the NSF charge and the balance owing on any other cheque etc. issued to MDMHA before the player is allowed on the ice.

Millbrook minor hockey will provide players until Oct 1st to request a refund, such refund to be prorated based upon available ice time to the point of cancellation and less administrative fees and any direct costs incurred by the association on behalf of the player. Any requests received after Oct 1st must prove extenuating circumstances (such as but not limited too a major injury or surgery requirement) and be approved by the board and will also be subject to proration as above should the board approve the withdrawal from play.
